Billing and Insurance

Fees

The Speech and Hearing Clinic is a non-profit organization. We provide services and products at reasonable rates. Fees for our services are used to support and maintain the operation of the Clinic.

We are able to provide a sliding fee scale for services for patients with documented financial need. Qualification for reduced fees is based on total family annual income and the number of dependents in the household. Students who are registered at Illinois State University receive a 50% reduction of all fees for services. Students must be registered during the term in which they are receiving services to qualify for the reduction in cost for services.

All patients are expected to pay for speech, language, and hearing services and products upon receipt of a bill. Payment for any balance is due in full prior to the start of the new semester or summer session.

Referrals

Based on your individual insurance, a referral for our services may be required. Please note that patients who qualify for Medicaid must have referring physician who is classified as a Medicaid provider. This referral must be on file with our office before any services can be provided. It is the patient’s responsibility to request this referral from their primary physician.

Insurance

Primary and secondary insurance, where applicable, will be billed for services. Any co-insurance payments/deductibles will be the responsibility of the patient and due at the time of service.

Private-Pay Patients

Patients who are uninsured, have an insurance provider not accepted by our Clinic, or whose insurance does not cover our services are classified as private-pay patients. All private-pay patients are responsible for payment at the time of service. Our office will provide you with an individualized good-faith-estimate of costs prior to receiving services. Read more information about your rights associated with the good-faith-estimate. Additionally, we will provide an invoice to the patient. The patient can submit this invoice to their insurance for reimbursement.

Forms of Payment

We accept payment in the form of check, or credit card. Current patients have access to their patient portal. Remittance for services can be submitted through this portal.

Patient Portal

When you scheduled your first appointment, you should have received a username and password for our portal. If you didn’t get this information, please contact our clinic, and we'll be happy to assist you.
